本課程主要是結合進階社區護理實習，協助學生對未來論文撰述之相關議題，引用理論深入之研討及批判。課程中，學生須與該專業領域相關專長的老師討論評估及介入的社區健康問題，並應用評估工具以確立社區健康問題，訂定成效指標，進行介入措施，再依據成效指標評值執行成效及困境。透過個別輔導達成學習目標，並於期中及期末時提出報告，同學參與本課程後將能提升自我對社區健康問題的評估、批判性分析、診斷、社區資源運用、介入措施執行與評價的能力，參與社區健康照護的永續發展。

　　The purpose of this practicum is designed to enhance graduate students’ community health assessment competence for taking care of community population. Student experience will focus on data collection and data analysis using various quantitative and qualitative approaches to community assessment. At the end of the course, the students will be able to develop the concept of community assessment, define the community health diagnosis and set the priority, integrate the Community resources and establish the partnership with the community.
