本課程主要是結合課式教學之社區健康評估與衛生計畫、進階社區護理學(I)以及進階社區護理學(II)之理論與知識；協助學生認識與學習與社區中不同組織或群體合作，共同營造社區健康。透過社區健康評估、確認社區健康問題、共同計劃執行健康處置並評值其成效。學習發展及撰寫社區健康計畫專案，並在該社區中永續經營。藉此過程統整社區護理理論與相關概念與實作，以展現進階社區護理師之角色及能力於社區群體之三段五級預防照護中。

　　The purpose of this practicum is designed to enhance the advanced community health nursing competence of graduate student. Student will apply the theory and knowledge that obtained from Community Health Assessment and Health Plan, Advanced Community Health Nursing (I), and Advanced Community Health Nursing (II) to understand and practice cooperative skills with different organizations or aggregate. Students will complete or initiate health-related intervention program as well as demonstrate the advanced community nurse’s role and functions in the community/aggregate.
